Personal Health Plan, you’ll receive a $100 Visa gift card per insured employee.
For businesses that choose a fully insured group benefits plan, you’ll
receive a $500 Visa gift card for groups with 2–5 employees, or a $1,000 Visa gift card for groups with 6 or more employees.
Gift cards are issued once the referred business becomes an active client.
